Having issues with Win10 Pcs using Sophos AV - can work fine for a number of days but then unable to access with a "Need Host Checker" error message in the user logs.
What would be preventing hostchecker from installing and/or running?

This can be a number of things. You may want to try and see if the Sophos itself it causing the issue, or blocking from running certain process.
You may want to look into the Process Monitor logs and see why it stops running.
