FYI I believe this is due to having multiple monitors and then switching to a different number. I've had this on my laptop and my desktop (moved from 3 to two different screens) Popup window gets lost. Its a pretty standard occurance on windows apps. I'd bet that it is the problem.
I don't have the exact reproduction steps, but, the scenario would be something along the lines of plug in multiple monitors, (this could be an external monitor for laptop) do some logins on that screen. Then shutdown, disconnect that screen, try login and the window is now off screen.

Having the same issue after upgrading to the latest windows patch. I tried bringing the task to the front, but it did not work.

I am hitting the same issue with logging window not popping. Tried that "Bring to front" option in task manager, but it did not do anything.
