Customizing UAC auth page

I have a client using UAC via an IC6500 to secure network access. Clients are redirected to a login page served up by the UAC via an enforcing SRX. 

Regarding customization of the login page, can it be configured for anonymous auth, however can it promt the user to enter details which are then saved to a file?  For example, the user would be prompted for their name and company. Once they enter this and click 'accept', UAC marks them as accepted.

Yes your requirement of customizing the loginpage of UAC is possible.

You can use custom signin pages options in UAC to achive your requirement, below is the URL for accessing the custoemr signin page guide which you or your IT team can use it for creating custom signinpages as per your requirement
